Certified Service Quality Certification Request Form

Please complete the following fields to submit the official request for your Certified Service Quality Associate designation.

Notes:

  • Your CSQA Candidate Number is the 7-digit number in the order number provided on your enrollment receipt. Eg. ORD1234567
  • Once submitted, your request may take up to 3-5 business days to be reviewed and approved. An email will be sent to you if any additional clarification or supporting information is required.
  • Due to Covid-19 restrictions, upon approval, a digital copy of your certification will be sent to the email address listed in your Candidate profile.
  • Once approved, employers can validate your CSQA certification status using the CSQA Verification Tool.
  • Certification Qualification Requirements:
    • A secondary degree (high school diploma, associate’s degree or the global equivalent) + 3 years of customer service related experience;
    • OR: a Bachelor’s degree (or the global equivalent) + 1 year of customer service related experience.
